1. Click on the 'Form Creation' option to proceed.

2. Include all necessary question types in your form to gather the required information as specified in your document.

3. Click on "Customize PDF"

4. Click on "Upload your PDF"

5. Select all the Questions from your form and adjust accordingly.

6. Click on "Preview" in your Form

7. Fill out the form.

8. Click on the "Eye icon" at Preview the PDF as you fill the form!"

9. You can now preview your PDF with the questions added to the form to check how it looks.

10. Click on "View & Verify PDF"

11. The pdf has been sent to the email and the response has been recorded.
