Introduction
Sometimes a form response is not complete with just text fields, signatures, or checkboxes.
You may need recipients to upload multiple images too.
For example, a customer may need to upload product photos, ID proof images, property images, damaged-item photos, event photos, medical documents, inspection images, or any other visual evidence.
With BoloSign’s Multiple Image Map Form Custom PDF feature, you can collect multiple image uploads through a form and automatically place those images inside a customized PDF.
This helps you turn every form submission into a complete, professional PDF document with the uploaded images included in the right place.
Steps to Add Multiple Images to a Custom PDF
Step 1: Open your form in the editor
Open your form inside the BoloSign form editor.
This is where you can add questions, file upload fields, signatures, and other form inputs.
Step 2: Add a File Upload field
Add a File Upload question to your form.
You can name it based on what you want recipients to upload, such as Upload Images, Attach Product Photos, or Upload ID Proof.
Step 3: Enable multiple image uploads
Turn on the Allow Multiple Files option.
Then, under The type of file users can upload for this question, select Only Images.
This allows recipients to upload multiple image files in the same field.
Step 4: Set the file size limit
Choose the maximum file size allowed for uploads.
For example, you can set it to 5 MB or another available limit based on your requirement.
Step 5: Open Customize PDF
Once your form fields are ready, click on Customize PDF.
This opens the PDF editor where you can map form responses to your custom PDF.
Step 6: Select the Multiple Images field
In the PDF editor, open the Pick Question from your Form dropdown.
Select the image upload field marked as Multiple Images.
For example:

Step 7: Place the image field on the PDF
Add the selected image field to the area where you want the uploaded images to appear.
You can adjust its size and position based on your PDF layout.
Step 8: Save and share the form
Click Save after mapping the fields.
Now share the form with recipients.
When they submit the form with multiple images, BoloSign will automatically add those images to the customized PDF.
Why This Feature Is Important
This feature is useful when your workflow depends on image-based proof or documentation.
Instead of collecting form responses and images separately, BoloSign brings everything together in one final PDF.
It helps teams save time, reduce manual work, and keep every submission organized.
This is especially helpful for:
Real estate teams
Insurance workflows
Field service teams
Event businesses
Healthcare teams
Education institutions
Legal and compliance teams
Inspection and maintenance teams
The final PDF becomes easier to review, approve, share, and store.
Conclusion
BoloSign’s Multiple Image Map Form Custom PDF feature makes it easy to collect multiple images from recipients and place them directly into a customized PDF.
You simply add a file upload field, allow multiple files, set the upload type to images, map the field in Customize PDF, and share the form.
Once the recipient submits the form, their uploaded images are automatically added to the final PDF.
This makes the entire process faster, cleaner, and easier for teams that rely on image-based records and approvals.
