1. Click on the "Actions icon" of the form you want to delete

2. Click on "Delete Form"

3. Click on "Delete" to delete the form

4. Click on "Responses" on the form where you want to delete your response

5. Click on "Delete" on the response you want to be deleted

6. Click on "Delete" to delete the response
